Membership Requirements:
Age 50+ and Completion & Signature of:
- Membership Application
- Code of Conduct
- Waiver of Liability
- Physician Release for Cardio &/or Strength Participation (as necessary)
- Payment of dues
HOW TO BECOME A MEMBER
Fill out and Print the following forms. Bring them with you to the Blaine Senior Center front office to take a tour, get registered and pay your dues.
